The aim is to reduce the contraction of the lateral platysma muscle fibers using botulinum toxin.
Contraction of the lateral platysma pulls downward on the sagging tissues of the cheeks. Over time, resting muscle tone increases, the lateral platysma shortens, and it no longer maintains the cervicomandibular support angle. As a result, the submandibular gland descends, the jawline loses definition, and jowls begin to appear.
Technique
A few units of botulinum toxin are injected on each side:
Along and just below the mandibular border
Into the upper portion of the lateral platysmal bands
Injection of the DAO (Depressor Anguli Oris) muscle should be combined with treatment of the underlying lateral platysma, as the motor end plates extend to this level.
When the treatment is effective, the mandibular border becomes better defined, resulting in an improvement of the jawline contour and lower facial oval.
